The Pediatric High-Risk Score (PHRS) was developed to predict aneurysmal subarachnoid hemorrhage (aSAH) in children. We hypothesized that the PHRS might also help to identify children at increased risk for aSAH due to cerebrovascular malformations (CVMs) and therefore might be useful in the treatment decision-making process. We retrospectively analyzed the data of a consecutive cohort of children with CVMs to compare the predictive performance of the PHRS and the modified PHRS (mPHRS) in the prediction of aSAH and mortality. In addition, we studied the performance of the modified PHRS to identify children with CVMs at risk for aSAH and recommend endovascular therapy. The study cohort included 246 children. Of these children, 86 (35.3%) had aSAH. The PHRS demonstrated a specificity of 82% and a sensitivity of 33% (AUC 0.620) for prediction of an aSAH. The mPHRS showed a specificity of 82% and a sensitivity of 60% (AUC 0.821).
